From 1 July 2025, Medicare made important changes to how people living with ongoing health conditions can access allied health services. These changes may benefit you — particularly if you have already used your 10 sessions under a Mental Health Treatment Plan (MHTP) in this calendar year.
Under the new GP Chronic Condition Management Plan (GPCCMP), you may now be able to access further psychology sessions if you are eligible. At Allied Health for Wellness, we want to ensure you are fully informed so that your chronic condition management continues smoothly with as few barriers as possible.
What Has Changed?
One plan instead of two
The previous GP Management Plans (GPMPs) and Team Care Arrangements (TCAs) have been combined into a single plan: the GP Chronic Condition Management Plan (GPCCMP).
Simpler referrals
Your GP no longer needs to coordinate with two or more allied health professionals to put the plan in place. Referrals are now provided in the form of a letter, as long as you have had a GPCCMP prepared or reviewed within the last 18 months.
Additional support
If you have used all sessions available under your MHTP, you may be eligible for an additional five allied health sessions per calendar year through the GPCCMP. This can include psychology.
Continuity for existing plans
If you already have a GPMP or TCA, these remain valid until 30 June 2027. New referrals after 1 July 2025, however, must follow the GPCCMP requirements.
Eligibility and Reviews
To use a GPCCMP for allied health services, you must have:
- A GPCCMP that was prepared or reviewed in the past 18 months.
- A referral letter from your GP that outlines the services recommended.
Your GP can prepare a new chronic disease management plan annually or review an existing one every three months if your health needs change. Regular reviews ensure your plan stays relevant to your current care goals and supports effective chronic condition management.
Medicare Rebates
The current Medicare rebate for psychology under a GPCCMP is $61.80 per session.
Please note that most allied health providers set their own fees, which means there will usually be a gap payment.
What This Means for You
This simplified approach to chronic condition management helps you access the support you need more easily, without unnecessary administrative steps. If you are managing a chronic condition and have already used your MHTP sessions, the GPCCMP may help you continue accessing psychological care without delay.
Next Steps
- Check with your GP: Ask if you are eligible for a GPCCMP or if your existing plan needs a review.
- Discuss your referral: If you qualify, your GP will provide a referral letter for psychology or other allied health services.
- Bring your referral with you: This ensures we can process your sessions promptly and you can continue your care with us.
